Yes I did in fact receive mine. And what I find odd about this claim form is a small change in options. I remember before that one of the options were to keep the verizon service and receive a credit for a new phone up to the purchase price. On this new claim form it doesn't mention anything about a credit, only refunds...

Final Hearings on the proposed settlement have been scheduled for January 17, 2006 (class members included in the initial mailing), and March 16, 2006 for class members who were included in the supplemental mailing. This website will be updated shortly thereafter, so please check back for additional information on the settlement.

I believe i was included in the initial mailing - so im set, correct?

We are both in the same boat. I already sent in the older one, but I assume since the documents have been changed you should send in the new one as well.

Yes, now I have to decide what to do. One of my v710's has water damage, so that's a no brainer. I'll get the refund. I just don't know whether it's worth dropping my Verizon contract with one year left. I've been very pleased with customer service in my area(Fresno, CA). The reception at my house is spotty, so I'm considering switching to Cingular.
