Hey has anyone received their phones yet that had to fill out a W9 form? I'm trying to figure out if I'll get mine by the end of this week. If you did have to fill out a W9, what was the timeframe from when you filled it out, to when you received your phone.

I had to fill out the W9 last week, and have yet to receive my phone. They are all supposed to be shipped by 11/18, unless you are supposed to fill out the W9 and haven't done it.
